Cagrilintide (AM833) is a long-acting, acylated analog of amylin — a 37-residue peptide hormone co-secreted with insulin. It was described by Kruse and colleagues in 2021 as a non-selective agonist at amylin and calcitonin receptors.

Design
Native human amylin aggregates into amyloid fibrils. Cagrilintide’s sequence substitutions reduce fibrillation, and a C20 fatty diacid side chain provides albumin binding for an extended half-life — a design shared with incretin analogs such as semaglutide.
Receptor Pharmacology
Amylin receptors (AMY1–3) are formed when the calcitonin receptor (CTR) associates with receptor activity-modifying proteins (RAMP1–3). Cagrilintide activates AMY receptors and CTR in cell-based cAMP assays.
